Simplified presentation of information is unique and promotes appropriate management decisions, and protocol driven ordering programming supports reliability and safety of care. The use of a complete patient-specific cluster of process and outcome measures, rather than evaluation of the process and outcome measures for specific diseases considered in isolation of co-existing conditions, brings a patient centered focus to the management of these conditions. The disease management interface system is independent of a specific proprietary electronic health record, but versions will be programmed for use with all electronic health records.
